Registration

We ask that new patients fill out and submit a registration and medical history PDF form. This form can be saved and submitted by e-mail or printed and delivered in person at the first session.

TWO IMPORTANT POINTS:

  1. Please use the Adobe Acrobat Reader, which is free for download, to fill out and save the registration form. Many programs, including the popular Chrome browser, do not allow you to save PDF form contents to a file. You will avoid much frustration by using the Adobe Acrobat Reader.
  2. Be absolutely sure that you are using Adobe Acrobat Reader when you start filling out the form. Look for the Adobe Logo and product name:Registration 1
    If you are using the Chrome Browser, it is particularly easy to open a downloaded PDF in Chrome by accident. To avoid having this happen, right-click on the downloaded file at the bottom of the browser screen and choose “Show in folder”:

    Doing so will give you more control over which program the PDF is opened in.

Scheduling an Appointment

We are using a new scheduling service called Schedulicity. Your name and email address may already be in the system,  but you will need to sign up as a new patient under the new system.
